Proof of your marital status. If you are married, provide a Marriage Certificate. If you are divorced, provide a Divorce Decree. If your spouse has passed away, provide a Death Certificate.
Proof of residence.
Proof of income, and a bank statement covering the last 3 consecutive months.

How to Apply

Application forms are not available online, so you complete the form at your nearest SASSA office in the presence of a SASSA officer, who can help you if you are unable to fill it in yourself.
